Registration

By signing below, I agree to the following:

• I give permission for my child to participate in the Legacy Mission Village's Journey Program.
• I understand that my child's school may contact Legacy Mission Village (LMV) with questions or concerns about my child and that LMV will communicate these issues to me. I also agree that LMV may contact my child's school or teachers on my behalf.
• I give permission for LMV staff to access my child's attendance records, grades, and Individualized Learning Plans (if applicable) or communicate with school personnel.
• I give permission for LMV representatives to drive my child to and from youth events, school, or other activities.
• If my child is injured while under the care of Legacy Mission Village and requires the attention of a doctor, I consent to, and will be responsible for, any reasonable medical treatment as deemed necessary by a licensed physician. I also agree to hold the licensed physician, the medical facility, Legacy Mission Village and its representatives free and harmless of any claims, demands or suits for damages arising from the authorization and provision of such medical treatment.
• I understand the nature of LMV programs and release Legacy Mission Village and its representatives from any liability due to accident or injury incurred by my child.
• I agree to cover all costs and/or arrange transportation if my child needs to be sent home for disciplinary reasons.
• I give permission for my child's likeness, voice, art or written work to be photographed, videotaped, or recorded for use in publications, media, advertising, or the annual report of Legacy Mission Village. I understand that LMV provides no compensation for such use. I understand that my signature below releases LMV's agents and employees from any and all liability stemming from the use of said images and recordings.

By signing below, I agree to the following:

• I will be respectful of Journey staff, volunteers, and students at all times.
• I will use appropriate language at all times while participating in Journey activities and message groups.
• I will leave the spaces in Journey as I found them, including computers, chairs, and cleaning up my trash.
• I will tell Journey staff or volunteers when I have a problem with another student if I don't think I can handle it respectfully myself.
• I will maintain a positive attitude at all times.
• I will do my best to improve my grades, behavior, and attendance in school.
• I understand that if I physically harm or assault Journey staff, volunteers, or students; I may be asked to leave immediately without warning and a parent conference will be required to determine if I am eligible to return for any activities. For severe incidents, police or other authorities may be notified.
